Description:
Large, mature myeloma cells are observed at the top of each hemiframe. Below are tri- and binucleate forms, also mature. In the former are two small fragments of DNA, possibly representing a stray chromosome. In the latter are a number of cytoplasmic Russell bodies, . See comment.
Comment:
Russell bodies are spherical, protein-rich dilations of rough endoplasmic reticulum.Their occurrence should therefore be limited to cytoplasm as they are in this case. Not so, however : they are also occasionally detected in nuclei of plasmacytoid cells. Very rarely, large numbers of Russell bodies are found in these nuclei (Multiple myeloma with numerous intranuclear Russell bodies. Martin-Noya A, Rios-Herranz E and Rafel-Ribas E. Haematologica 1999. 84: 179-180). In neither cytoplasm nor nucleus are they diagnostic of malignancy since they are also occasionally encountered in normal or reactive plasma cells. Compare these inclusions with Dutcher bodies illustrated in an upcoming case.
