Description: 
In this the final frame of the case, two mature myeloma cells are shown. A quadrinucleate variant is shown on the left. Whether the cell on the right is a quadrinucleate or a bilobated binucleate variant is not quite clear. See comment.
Comment: 
These three compos-ite photomics are from another case of putative pleomorphic myeloma. The author has no further informat-ion as to history, isotype,or how many of these cell were present in marrow smears or biopsy. If ‘numerous’, this would be classified as a pleomorphic myeloma. If so, as pointed out in the preceding case, it would then behave as a myeloma exhibiting intermediate aggressiveness. Light chain myelomas (here κ-chain) are also known to be more aggressive than standard (‘major’) forms of the disease, even in the abscence of pleomorphism
