Note the contrast in size between this Brobdingnagian myeloma cell and the more standard - sized myeloma cell below it . Note the fairly large nucleolus in the former.. Shown for contrast on the far right is a normal late polychromatophilic normoblast. See comment.
Comment: 
The patient’s first bone marrow study was performed in 1969. The author has no data on that marrow. The present marrow study was performed in 1972. Despite the ominous appearance of her neoplastic cells, she had lived for at least three years with the help of the standard chemotherapy of the day. Autologous transplantation had not yet begun. It is not surprising that this case of light chain myeloma manifested with bizarre, primitive morphology. Light-chain myelomas tend to exhibit fairly aggressive behaviour, pleomorphic morphology or both. They still however do not approach the feral belligerance of either anaplastic or plasmablastic myeloma
