Bx: Even in section, the appear-ance of some myeloma cells could give the impression of a monocytoid disorder . Many cell nuclei are deeply indented , lobulated or even binucleate. See comment.
Comment:
Nuclear indentations leading to a monocytoid appearance and hyperlobulation of myeloma cells as illustrated in both photomicrographs in this case, have been described by transmission electron microscopy. Both features have been ascribed to the presence and / or constrictive action of tiny cytoplasmic juxtanuclear micro-filaments. (Phagocytic, Lambda Light Chain , Plasma Cell Myeloma..Am J Clin Pathol: Wirt D P, Grogan TM, Payne CM 1983; 80 (1): 75-84). The isotype of the present case is not known to the author. END OF CASE.
