MARROW SMEAR
Description: 
This is a marrow smear from a 67 year old woman on long term treatment with methotrexate for rheumatoid arthritis. For lab studies see adjacent column. The field shows a large number of generally mature plasmacytoid cells. Nucleoli are small and inconspicuous.
Comment: 
A diagnosis of smoldering hemophagocytic myeloma was rendered by the controbutor of this case. Based on the M-spike of 2.1gms, the author would have to agree.

LAB : The only laboratory data available are as follows: increased serum total protein, increased ESR, abnormal SPEP showing monoclonal M spike and increased β2 microglobulin . The M-spike proved to be 2.1 gms of IgGκ.
